Support Services for Older People (Brimhay)
 

This is a quick reference card to give details of the services provided to residents under the Supporting People Regime. List is not exhaustive, but covers the main service areas and responsibilities. The Support Worker will normally work between the hours of 9am and 12 noon, Monday to Friday. Some time will be set aside each day to accommodate administration and other duties relating to the provision of services.

 
We will:
 

Ö        Visit you or contact you each day Monday to Friday (except Bank Holidays)

Ö        Ensure that assessments of your needs and risks are carried out on a regular basis and take into account your views, involving carers and other        professionals.

Ö        Liaise with other care and health professional to enable you to access care and support according to your needs.

Ö        Help you ensure security, health and safety is protected.

Ö        Request a Doctor, Health Worker or other Health or Care professional to attend if you require help.

Ö        Arrange delivery of or collect medication if you are unwell

Ö        Transmit requests for repeat prescriptions.

Ö        Arrange for meals to be delivered if required.

Ö        Keep you informed of community and social events.

Ö        Keep the service as flexible and sensitive as possible to maximise your dignity, independence choice and control over your life.

Ö        Through a Quality Assurance framework, promote continuous improvement.

Ö        Maintain your right to be protected from abuse.

Ö        Help you to arrange for hairdresser, optician, dentist or chiropodist wherever possible.

Ö        Provide welfare advice and assist in filling forms where possible.

Ö        Preserve confidentiality at all times.

Ö        Consult you on changes to the service.

Ö        Give occasional help with collection of shopping or pensions if you are unwell.

Ö        Ensure you have access to and knowledge of our complaints procedure.

When the Support Worker is not on duty all residents will receive the Emergency Community Alarm Service provided by Forder Lane House as detailed below:

 
 

Ö        Deliver an out of hours service 24 hours a day 365 days of the year.

Ö        The system is intended for genuine emergencies only.

Ö        We will respond to all calls made on the system treating them as emergencies and respond within our target time of 60 seconds.

Ö        All activations of calls are recoded with comments on the outcome.

Ö        We will call the emergency services, a Doctor or other contact or family member according to the nature of the emergency.

Ö        We will keep in contact with you until help arrives.

Ö        In the most serious of cases a member of staff may visit the caller to assess the situation and comfort the individual until professional help         arrives.

Ö        Check and maintain the alarm system on a regular basis.

Ö        Use the call system to check on the welfare of service users when the Support Worker is not on duty, where the Support Worker has agreed this is necessary.

 
We are not able to:
 

´       Undertake domestic work

´       Undertake support or care of a personal nature

´       Administer medication

´       Undertake any Housing Management duties
